Histamine 101: What It Is, What It Does, and Why It Can Make You Feel Awful

If you’ve spent any time in the wellness world lately, you’ve probably heard the word histamine thrown around.

Maybe you’ve seen a list of “high-histamine foods.” Maybe someone told you tomatoes are the problem. Or leftovers. Or avocados. Or fermented foods.

And suddenly you're standing in your kitchen wondering what exactly you're supposed to eat.

Before we start removing half the refrigerator, though, we need to understand something important:

Histamine isn't inherently bad.

Your body actually needs it.

The issue may arise when the amount of histamine your body is dealing with exceeds its ability to break it down efficiently.

So let's start at the beginning.

What Is Histamine?

Histamine is a naturally occurring chemical messenger produced by the body. It plays several important roles, including supporting immune responses, digestion, blood vessel function, and communication within the nervous system.

Most people associate histamine with allergies.

You encounter pollen, pet dander or another allergen, your immune system responds, histamine gets released, and suddenly you're sneezing, itching or reaching for the tissues.

But histamine isn't only involved in allergies.

It also participates in things like:

  • immune and inflammatory responses
  • stomach acid production and digestion
  • regulation of blood vessels
  • sleep and wakefulness
  • communication within the nervous system

In other words, the goal isn't to eliminate histamine from your body.

You couldn't, and you wouldn't want to.

The goal is balance.

Where Does Histamine Come From?

Your total histamine exposure can come from several places.

First, your body produces histamine naturally. Much of it is stored in immune cells called mast cells and basophils. When the body receives certain signals, those cells can release histamine.

Second, histamine can also come from food.

Histamine levels tend to be higher in certain foods, particularly foods that have been aged, fermented, processed or stored for longer periods.

This is one reason someone may tolerate a freshly cooked meal but feel completely different after eating certain leftovers.

It isn't necessarily the chicken that changed.

The chemistry did.

How Does the Body Get Rid of Histamine?

Thankfully, the body has systems for breaking histamine down.

One of the most commonly discussed enzymes is diamine oxidase, or DAO.

DAO is particularly important for helping break down histamine in the digestive tract, including histamine coming from food.

Another enzyme, called histamine N-methyltransferase (HNMT), helps metabolize histamine inside cells and tissues.

Think of histamine like water entering a bathtub.

Your body can normally handle the water because the drain is open.

But if more water starts entering the tub while the drain isn't keeping up, eventually the water level rises.

That's essentially the theory behind what is commonly called histamine intolerance.

It's less about histamine being the villain and more about the relationship between your total histamine load and your body's ability to manage it.

What Is Histamine Intolerance?

Histamine intolerance is a term used to describe symptoms thought to occur when histamine accumulates faster than the body can adequately metabolize it.

Importantly, histamine intolerance is not the same thing as a food allergy.

Food allergies involve a specific immune response to a food protein. Histamine-related reactions can be much less straightforward.

And this is where things get messy.

Histamine intolerance is still an evolving area of research. There isn't currently one universally accepted laboratory test that can definitively diagnose it, and symptoms can overlap with allergies, gastrointestinal disorders and several other conditions.

That's why symptoms alone shouldn't automatically lead to a diagnosis of histamine intolerance.

What Can Histamine Symptoms Feel Like?

Because histamine receptors exist throughout the body, symptoms associated with excessive histamine activity can appear in several different systems.

People may report things such as:

  • headaches or migraines
  • flushing
  • itching or hives
  • nasal congestion or runny nose
  • bloating
  • abdominal discomfort
  • diarrhea
  • nausea
  • dizziness
  • changes in heart rate or palpitations

And that's part of what makes histamine so frustrating.

One person eats a particular food and gets a headache. Another gets flushed. Someone else looks six months pregnant after dinner. And someone else eats the exact same meal and feels completely fine because apparently human bodies needed another opportunity to be unnecessarily complicated.

Why Would Someone Have Trouble With Histamine?

There probably isn't one universal answer.

Researchers are continuing to study the role of DAO activity, genetics, gastrointestinal health, medications, alcohol, dietary factors and other influences on histamine metabolism.

This is also why I don't love the idea of immediately labeling every unexplained symptom as “histamine intolerance.”

Symptoms are information.

They're telling us something is happening.

The better question is:

Why is this person reacting in the first place?

That's much more useful than simply creating an increasingly tiny list of “safe” foods.

What About High-Histamine Foods?

This is where many people first discover histamine.

Foods commonly discussed as higher in histamine include aged cheeses, fermented foods, processed meats, alcohol and certain fish. Histamine content can also be influenced by freshness, storage and processing.

But here's the important part:

A low-histamine diet isn't necessarily meant to become a forever diet.

Over-restricting food without understanding the bigger picture can create an entirely different set of problems.

For some people, a temporary reduction in higher-histamine foods may help identify patterns. Ideally, foods are then systematically reintroduced to determine individual tolerance rather than assuming every food on a high-histamine list needs to disappear permanently.

Because nobody needs another spreadsheet telling them they're somehow reacting to spinach, avocado, yogurt and the concept of happiness.

Histamine Is More About the Big Picture

If you remember one thing from this introduction, make it this:

Histamine itself isn't the enemy.

It's a normal chemical messenger your body uses every day.

The real question is whether your body is producing, receiving and clearing histamine in a way that keeps the system balanced.

And when that balance gets disrupted, figuring out why matters.

That's where the conversation gets much more interesting.
